How we assemble operator pages

Grouping starts from the operator and legal names stored in public federal datasets. Nursing homes are grouped by their CMS legal business name; other care and housing types are grouped by the provider or property name. Normalization is limited to letter case and whitespace. Government licensing entities such as public hospital districts and county hospitals are filtered out, so unrelated private facilities are never combined into a chain that does not exist.

The counts, care types, and states shown for an operator reflect the data available at processing time and shift as facilities open, close, or change hands. For ownership and program participation today, check with the provider directly and at Medicare.gov.

Here, an operator is the company, health system, or organization responsible for running one or more care facilities that appear in public federal data. Facilities sharing an operator name are collected onto a single page so a chain's locations can be reviewed side by side.

Grouping starts from the operator and legal names stored in public federal datasets (CMS Care Compare, the CMS NPI registry, and HUD; processing date 2026-10-06). A name qualifies for its own page only when at least 3 facilities of the same care type carry it, which keeps single-site providers out of this index.
